Princess Catherine has spent much of September 2026 steadily returning to a fuller schedule of royal engagements, appearing everywhere from a glamorous London movie premiere to community visits across England.
Yet amid the attention surrounding her clothes, her causes and her public comeback, an unexpectedly small detail has become the subject of online conversation: her smile.

Recent photographs of the Princess of Wales smiling broadly have prompted some social-media users to ask whether her teeth or lips look different from before. Close-up images and different angles have fueled comparisons with older photographs, with comments ranging from curiosity about her teeth to claims that her lips appeared unusually thin in certain frames.
But there is an important distinction between what photographs actually show and what viewers may infer from them.
There has been no verified public announcement that Catherine has recently undergone dental work or a cosmetic procedure affecting her lips. And individual photographs—especially frames captured in the middle of speaking, laughing or changing expressions—cannot establish that something medically or cosmetically “happened” to her face.
What the latest images do reveal is a Princess of Wales who has been smiling frequently during an unusually busy stretch of public appearances.
A series of new appearances put Catherine back in close-up
September has brought a noticeable increase in Catherine’s public schedule.
On September 22, she joined Prince William at the world premiere of Digger in London’s Leicester Square, where the couple appeared alongside Tom Cruise and other guests. Catherine wore a plum Jenny Packham gown and was photographed repeatedly smiling for cameras. InStyle

The photographs were widely circulated.

Only days later, she was back in public again.

Then came engagements on September 29 and 30, including an appearance with William at Headingley Rugby Stadium and a solo visit connected with West Sussex Minibus.
Each event produced dozens of photographs.
And when someone is photographed continuously—smiling, speaking, laughing, listening and reacting—some frames inevitably look dramatically different from others.
One smile can look surprisingly different from the next
Anyone who has ever paused a video at exactly the wrong moment knows how much a face can change within a fraction of a second.

A broad smile stretches the lips horizontally.
The upper lip can consequently look much thinner than it does when the face is relaxed.
The amount of visible gum and tooth can also change depending on the intensity of the smile, the position of the jaw and whether someone is speaking at the instant the photograph is taken.
Camera angle matters too.

A photograph captured slightly below someone’s face can produce a different impression from one shot directly ahead. Harsh daylight, shadows and high-resolution telephoto photography can further emphasize details that aren’t particularly noticeable in person.
That makes comparisons between isolated photographs surprisingly unreliable.
Some viewers focused on her teeth
Catherine’s smile has actually been one of her most recognizable features throughout her public life.
For years, photographs have shown a wide smile with prominent upper teeth.
Recent pictures continue to show that.
What has changed is the sheer amount of attention now directed toward individual images of Catherine following a period in which her public appearances were significantly reduced.
At the Digger premiere, for example, photographs showed her smiling widely enough for both upper and lower teeth to become visible in some moments.
A black-and-white portrait released from the event similarly captured Catherine and William smiling broadly. Coverage of the photograph specifically noted the couple’s visibly bright smiles. PureWow

That does not, by itself, establish a dental change.
There is no confirmed explanation involving new dental work
Online discussions about celebrity appearances often move very quickly from observation to explanation.
A photograph looks different.
Someone asks why.
Another person suggests veneers, orthodontics, fillers, weight changes or a medical explanation.
Within hours, speculation can begin circulating as though it were established information.
In Catherine’s case, current reliable reporting does not provide evidence for a recent dental procedure that would explain the photographs circulating online.
There is likewise no verified statement from Catherine or Kensington Palace announcing such a change.
Without that information, claims about a specific procedure remain speculation.
Her lips also became part of the discussion
Another detail attracting attention was Catherine’s upper lip.

In some smiling photographs, it appears very thin—leading to dramatic online descriptions suggesting that it had nearly “disappeared.”
But photographs taken seconds apart can produce completely different impressions.
When someone smiles broadly, the upper lip naturally stretches across the teeth. Depending on the person’s anatomy and the intensity of the expression, substantially less of the lip may remain visible.
When the same person relaxes their mouth, the lip appears fuller again.

That’s why a photograph showing Catherine with a wide smile cannot reliably establish a permanent change in her lips.
And there is no credible public evidence confirming that she recently underwent a procedure to alter them.
Her appearance has received unusual scrutiny since her cancer treatment
There is another reason photographs of Catherine now receive extraordinary attention.
In March 2024, Catherine publicly announced that cancer had been discovered following abdominal surgery and that she was undergoing preventative chemotherapy.
Her public schedule became much more limited while she received treatment.
She later announced that she had completed chemotherapy, and in January 2025 she said she was in remission.
By September 2026, Catherine had returned to a much more visible schedule.
On September 11, she visited the Royal Marsden Hospital, where she had previously received cancer treatment, to see how funds raised through her charitable efforts were being used. Sky TG24

That history understandably means that changes in her appearance can attract attention.
But it also makes it especially important not to turn photographs into medical evidence.
A picture cannot reveal someone’s health status.
September showed Catherine becoming increasingly visible again
Rather than one isolated event, Catherine’s recent photographs document a series of engagements.

On September 10, she joined William in Liverpool wearing a cornflower-blue Edeline Lee suit. Vogue
The following day brought her Royal Marsden visit.
On September 17, William and Catherine traveled to Scotland’s Isle of Bute, where they participated in community activities and even tried the traditional sport of shinty. InStyle
September 22 brought the glamorous Digger premiere.
Two days later, Catherine visited the Forward Trust.
Then she appeared in Leeds on September 29.
And on September 30, she traveled to West Sussex for another solo engagement.
That’s a remarkably dense collection of recent photographs.
The September 30 pictures offered another comparison
Catherine’s West Sussex appearance is particularly useful because photographers captured her under ordinary daylight rather than movie-premiere lighting.
She wore a monochromatic gray ensemble consisting of a long coat, pale blouse and trousers, with burgundy accessories. Town & Country
She rode aboard a community minibus and helped collect passengers, including 94-year-old Stella Drewett, before joining people at a lunch club in Pulborough Village Hall. People.com
Images from the outing captured Catherine in numerous expressions.
Some show her smiling.
Others show her talking.
Others capture her mouth almost completely relaxed.
The differences between those photographs demonstrate precisely why one viral frame can be misleading when used to make claims about someone’s appearance.
She even joked about something much more mundane
During that same outing, Catherine shared an amusing detail about herself.
She admitted that she frequently forgets her handbag and sometimes has aides running after her to return it. InStyle
It’s a small anecdote, but it illustrates how different the actual engagement was from the intense scrutiny that can surround photographs afterward.
Catherine was meeting volunteers, talking with older residents and highlighting a community transportation service designed to combat isolation.
Online, meanwhile, individual images can become discussions about individual facial features.
Catherine has faced appearance speculation for years
This isn’t a new phenomenon.
Ever since Catherine Middleton entered royal life, virtually every aspect of her appearance has been analyzed.
Her hair.
Her eyebrows.
Her weight.
Her skin.
Her clothes.
Her makeup.
Her teeth.
Even relatively ordinary changes in hairstyle have generated international headlines.
The level of attention intensified following her health problems because there were fewer public appearances available for comparison.
When she returned, every new photograph inevitably became material for side-by-side images.
But side-by-side photographs don’t necessarily compare like with like.
Lighting can change more than people realize
Consider two photographs of the same person.
One is taken outside in direct sunlight with a long telephoto lens.
The other is taken indoors under soft lighting.
Even without makeup or any physical change, skin texture can look dramatically different.
The same principle applies to teeth.
Brightness, contrast, white balance and surrounding colors can make teeth appear whiter, darker, larger or more defined.
Digital compression can further alter fine details.
And celebrity photographs distributed online are frequently resized, sharpened or compressed multiple times before viewers encounter them on social media.
None of those factors proves that the viral photographs of Catherine were altered.
They simply illustrate why visual comparisons require caution.
Expression may be the biggest factor of all
The most straightforward explanation for some of the striking differences between Catherine’s recent photographs is also the least dramatic.
She moves her face.
During an engagement, Catherine may smile for photographers, immediately turn to greet someone, begin speaking, laugh and then return to a neutral expression.
A photographer shooting continuously can capture every fraction of that sequence.
Select one frame and her upper lip might appear extremely thin.
Select another half a second later and it may look completely ordinary.
That’s not unusual.
It’s simply how facial movement works.
The photographs from the Digger premiere attracted particular attention
The September 22 premiere provided especially high-quality close-ups.

She greeted Tom Cruise, who later described Catherine and William as “amazing” and “very charismatic.” InStyle
Numerous photographs show Catherine smiling throughout the evening.
Because she wore her hair away from much of her face and the event was heavily photographed, her expressions were especially visible.
Those images then became ideal material for social-media comparison posts.
A viral question isn’t the same thing as a verified change
“What happened to her teeth?” is a question.
It isn’t evidence that anything happened to them.
Likewise, describing Catherine’s lips as having “disappeared” reflects how they appeared to some viewers in a particular photograph—not a medical or cosmetic finding.
The distinction matters.
Recent reliable coverage documents Catherine’s engagements, clothing, conversations and continued return to royal duties.
It does not document a newly confirmed dental transformation or cosmetic lip procedure.
Without such confirmation, the photographs can tell us only what Catherine looked like at the instant each camera shutter fired.
Her smile has looked different across photographs for years
Looking back through older photographs reveals similar variations.
Sometimes Catherine gives a relatively restrained smile with little tooth showing.
Sometimes she laughs with her mouth completely open.
Sometimes her upper lip looks fuller.
Sometimes a broad grin stretches it until very little is visible.
Her facial expressions have never been identical from photograph to photograph.
What is different now is the degree of scrutiny.
Following an intensely difficult period in her personal life, Catherine’s return to regular royal appearances has made each new set of photographs unusually newsworthy.
She has spoken about recovery as a gradual process
Catherine herself has previously emphasized that returning to ordinary life after cancer treatment isn’t necessarily instantaneous.
That context is more meaningful than attempting to diagnose her through photographs.
Her September schedule nevertheless shows that she has resumed a substantial number of public engagements.
The Royal Marsden visit was particularly symbolic because it brought her back to the hospital associated with her own treatment. Coverage from the visit described her as being in remission. British Vogue
Her subsequent appearances have continued that gradual return.
Two photographs can tell two completely different stories
This may ultimately be why the viral images attracted so much attention.
One photograph can make someone’s smile appear completely different from another.
Freeze the exact moment the lips stretch over the teeth, and the upper lip can appear unusually thin.
Capture someone midway through speaking, and the teeth can look oddly positioned.
Use a different lens or angle, and proportions can appear different.
None of that means viewers are imagining the differences they see.
The photographs really can look different.
What they cannot establish on their own is why.
What is actually known
Catherine, now 44, has been photographed extensively during a particularly busy September.
She has smiled broadly at several engagements.
Some of those images have prompted online comments about the appearance of her teeth and lips.
But no reliable evidence currently establishes that she recently underwent dental or cosmetic work responsible for those appearances.
The strongest conclusion the photographs support is considerably simpler:
Princess Catherine looks different in different photographs because the images capture different expressions, angles, lighting and moments.
Anything more specific would require evidence beyond a viral photograph.
And amid all the online speculation, perhaps the more significant story is hiding in plain sight: after the health crisis that dramatically reduced her public schedule, Catherine has spent September appearing increasingly comfortable back in public—meeting communities, supporting charities and, yes, smiling for the cameras again.
